TDSB Occupational Illness Reporting

This document is to be completed by a worker to report an illness or disease that was contracted in the workplace.

Members are required to login to the TDSB website to access this document.

An occupational illness is a condition that results from workplace exposure to a physical (e.g. noise, vibrations), chemical (e.g paint fumes, roofing materials, hygiene materials), or biological (e.g. chickenpox, food poisoning), agent.

This form is completed by a worker to report an illness or disease that was contracted in the workplace.

The Principal is responsible for the investigation of any Occupational Illness, using the Supervisor’s Accident/Incident Investigation Report.
